Requestor will be required to complete a short form and present photo ID.
Unless otherwise advised, requested materials will be available within three to five business days after request is received.
You can download and fill out the required request form beforehand by clicking the link below or you can pick up a copy of the form at the Police Department. A PDF Reader is required to view the form. You must bring the form along with your Photo ID in person to the Records Department. You can also email the completed form and photo ID to the HPD Records Department.
